In the event your order is incorrect, the following guidelines will help you determine what options you have available.

• If Claim Forms Plus makes an error on an order, then
  • Claim Forms Plus will pay all shipping and product cost to correct the order.
• If the customer requires a refund/exchange, then
  • Claim Forms Plus will will credit the original purchase, less all shipping costs and a restock fee of $10 per box.
